Member Acknowledgement, Waiver, and Agreement
By signing this document, I acknowledge, agree, and consent to the following terms and conditions of participation at the Club (PK Taekwondo):
1. Compliance with Club Directions
I agree to comply with all reasonable directions, rules, and instructions issued by the officials, employees, and staff of the Club in relation to:

My entry to and exit from the Club premises;

The use of all facilities and equipment provided by the Club; and
-
My personal behaviour and conduct while on the Club premises.
2. Personal Property
I acknowledge that I am solely responsible for any and all personal belongings, including athletic equipment, brought onto the Club premises or to any activity conducted by the Club. The Club accepts no liability for loss of, or damage to, such items.
3. Medical Treatment and Fitness to Participate

I consent to receive any medical treatment that may be deemed necessary or advisable in the event of an injury, illness, or accident occurring on Club premises or during Club-related activities.

I acknowledge that it is my responsibility to seek medical advice and obtain clearance from a qualified medical professional prior to engaging in any physical exercise or use of Club facilities.

I declare that I am in good health and physically and medically fit to use the Club’s facilities. I am not aware of any condition that would place me or others at risk while engaging in physical activity at the Club.
4. Assumption of Risk

I understand and accept that participation in physical exercise and the use of gym and training equipment is inherently risky and may involve strenuous activity which can result in injury or harm.

I acknowledge that the Club does not and cannot guarantee the safety or suitability of any equipment, facilities, or training activities offered.

I voluntarily assume all risks associated with the use of the Club’s premises, equipment, and participation in Club-related activities.
5. Behaviour and Conduct
I agree to conduct myself in an orderly, respectful, and responsible manner while on the Club premises. I will not engage in any conduct that could endanger others, cause harm, or constitute a nuisance to other members or staff.
6. Orientation and Induction
I confirm that I have completed a full orientation and induction of the Club facilities, including instruction on the safe and proper use of equipment.
7. Transition to Online Services
I acknowledge that in the event of a government-mandated lockdown by the Queensland or Australian government, face-to-face classes may be suspended. In such circumstances, the Club will transition services to online platforms, including but not limited to Zoom or YouTube, and I consent to participate in this alternative format as required.
8. Direct Debit Payments
I acknowledge and agree to the following conditions regarding direct debit membership payments:

A minimum of four (4) weeks’ written notice is required to cancel any direct debit membership.

A minimum pause period of two (2) weeks is permitted for holidays or absences, subject to at least two weeks' prior written notice.
9. Make-Up Classes for Injury or Holiday Absence for Casual and Basic memberships
If a member is unable to attend scheduled classes due to personal injury or holiday leave, the Club may offer make-up classes at its sole discretion. Make-up classes are subject to availability and must be used within the following week or the week before the absence where possible. They are not guaranteed, not transferable to other members, and cannot be credited against membership fees. Make-up classes will not be offered to members who are absent without prior notice (“no-shows”). The Club reserves the right to limit or decline make-up classes and all decisions regarding eligibility and scheduling shall be final.
By signing below, I acknowledge that I have read, understood, and voluntarily agree to the above terms and conditions in full. I understand that this document is legally binding and affects my rights.
